List the training each role genuinely needs, then decide which records require evidence and renewal monitoring.
This avoids a tracker full of historic training items that nobody is responsible for reviewing. Give each role rule an owner and review it when duties, risks or local policy change.

Keep proof with the record
A completion date is easier to trust when the certificate or supporting evidence is attached to the same record.
If proof is missing, it should become an action rather than a note hidden in a comments column.
Review overdue and due soon records regularly
The best process gives managers a short action list: overdue training, due soon renewals and missing evidence.
Review new starters, leavers and role changes alongside that list; otherwise a clean-looking matrix can be based on the wrong population.
